Employer Authorisation Form

Dear Healthcard Employer,

During November 2024, employees have the opportunity to withdraw funds from their Healthcards to be paid into their personal bank accounts. However, this must be approved by employers before employees are informed of the withdrawal window period.

Should you wish to make the withdrawal option available to employees, kindly authorise the withdrawal by completing the form provided below. The Employer Authorisation form must be submitted by Friday, 8 November 2024.

Should employees be authorised to withdraw their funds, they will be required to complete the Individual Withdrawal form as well, and submit along with the required documentation.

The link to the Individual Form will be communicated to employees once authorisation from the employer was received.

Use of the Agility Healthcard is subject to the terms and conditions outlined in the employer’s agreement with Agility Healthcard, which may be updated from time to time.
